No planning problems says Children’s Hospital chiefs
Any planning problems with the new National Children’s Hospital are “entirely manageable”, the board has claimed.
The board of the facility, to be sited at St James’s Hospital, has moved to ease fears about potential planning problems at the site.
Concerns had been raised that the proposed new hospital — now due to be delivered by the later date of 2018 — may hit a planning hurdle for the second time.
The 6.3-hectare (15.5 acres) site has to be rezoned with the 108,000sqm building expected to be up to nine storeys high — around 10 metres above the maximum 28 metres allowed in the area. Local residents are expected to object. The cost of the original build on the Mater site was €650m but the project’s backers believe it can be built for €484m.
Around €39m had already been spent on the Mater site, with around €13m in design work capable of being carried over to the new site.
The facility would be built on the south inner-city campus after the original plans to erect it alongside the Mater Hospital were scuppered when An Bord Pleanala rejected planning permission.
A spokesman for the National Paediatric Hospital Development Board last night insisted the particular local planning issues in relation to the St James’s site can be managed.
“We are fully satisfied as has been already referred to by a number of expert sources that the planning issues are entirely manageable and take full account of the particular local environment in framing a plan for the new hospital,” the spokesman said.
The minister already pointed out he had sought a larger footprint on the St James’s site to reduce the height of the building to try to meet the restrictions on new builds.
A separate report from the Office of Public Works outlines other risks, including the need to upgrade the electricity and sewerage system. Junior Health Minister Alex White yesterday said all of the planning implications and difficulties had been examined as the new site was being selected over the past month.
He pointed out in addition to the Dolphin review group which assessed the various sites, planning experts John Martin and Simon Clear, a former inspector with An Bord Pleanala, also submitted a further report to the Health Minister on the planning issues associated with the particular sites.
Louis Roden, chair of the parents’ group, The New Crumlin Hospital Group, said “planning issues” would naturally arise with any site.
“What we need now is that the Government find a way to get the planners to contribute to the decision, I know this is not allowed at the moment,” he said.
“All medical advice says it has to be located with a main adult hospital. If you look realistically there were just a handful of options.”
Do certain types of foods affect Colon Cancer Survival?
Whole-grain foods and others with a low glycemic load may protect against colon cancer recurrence.
A new study suggests that what you eat may affect your chances of surviving colon cancer.
The research is among the first to look at the impact that specific nutrients have on the likelihood of disease recurrence in people with colon cancer, one of the leading causes of cancer death in the United States. It found that people treated for Stage 3 disease, in which tumor cells have spread to lymph nodes, had greatly increased chances of dying of it or experiencing a recurrence if their diets were heavy in carbohydrate-rich foods that cause spikes in blood sugar and insulin.
The patients who consumed the most carbohydrates and foods with high glycemic loads — a measure of the extent to which a serving of food will raise blood sugar — had an 80 percent greater chance of dying or having a recurrence during the roughly seven-year study period than those who had the lowest levels. Stage 3 colon cancer patients typically have a five-year survival rate of about 50 to 65 percent.
The study, however, was observational, meaning it could only highlight an association between carbohydrates and cancer outcomes without proving direct cause and effect. The researchers also obtained some of their data from food questionnaires that required patients to recall details about their diets, a method that can be unreliable.
Still, the researchers, who published their findings in The Journal of the National Cancer Institute, believe insulin may play a critical role in colon cancer recurrence. Chronically high insulin levels have been linked to cancer recurrence and mortality in previous research, and people with a history of Type 2 diabetes or elevated plasma C-peptide, a marker of long-term insulin production, have also been found to have an increased risk of colon cancer. One hypothesis is that insulin may fuel the growth of cancer cells and prevent cell death, or apoptosis, in cancer cells that have spread.
“It’s not simply that all carbs are bad or that you should avoid all sugar,” said Dr. Jeffrey A. Meyerhardt, the lead author of the study and an associate professor of medicine at the Dana-Farber Cancer Institute in Boston. It’s not as simple as ‘sugar causes cancer to grow.’”
He added: “Different carbs and sugar lead to different responses in your body. I think people should focus on a well-balanced diet” and substitute foods associated with lower glycemic loads or carbs for foods that have higher levels.
Earlier research published by Dr. Meyerhardt’s group showed that Stage 3 colon cancer patients who most closely followed a Western-style diet — with high intakes of meat, fat, refined grains and sugary desserts — had a threefold increase in recurrence and death from the disease compared with those who most strongly deviated from Western patterns of eating.
For this study, Dr. Meyerhardt and his team wanted to see to what extent carbohydrate intake could influence the progression of the disease, so they followed about 1,000 Stage 3 colon cancer patients taking part in a clinical trial sponsored by the National Cancer Institute. The patients, who had all had surgery and chemotherapy as part of their treatments, provided information on their diets and lifestyle habits. But the researchers went beyond just carbohydrate and sugar intake, taking into account glycemic measures.
The glycemic index, an increasingly popular nutritional measure, looks at the rate at which carbohydrate-containing foods raise a person’s fasting level of blood sugar and subsequent need for insulin. Sugary drinks, white bread and other highly processed carbohydrates rank higher on the index, while those that are digested more slowly, like brown rice, many vegetables, unrefined grains and legumes, have a lower index value.
Another barometer, however, is the glycemic load, which refers to the blood sugar effect of a standard serving of a food. A glycemic load of 10 or less for a food is generally considered low, while 20 or more is high. The latest study showed that glycemic load and total carbohydrate intake were the best predictors of cancer recurrence and mortality, and the link was strongest in people who were overweight or obese.
Dr. Meyerhardt said the findings suggest that colon cancer patients would be wise to keep glycemic load in mind while making food decisions, looking for ways to work into their diets foods that rank lower on the scale.
“So if you think about beverages, most juices and certainly sodas have a higher glycemic load than flavored waters and tomato juice and things like that,” he said. “Fruits like a date or raisins have very high glycemic loads, whereas fresh fruits like an apple, orange or cantaloupe all have sugar but have a very low glycemic load. Substitute brown rice for white, whole grains instead of white bread, and instead of having a starchy potato as your side dish, substitute beans and vegetables.”
One expert who was not involved in the research, Somdat Mahabir, a nutritional epidemiologist with the National Cancer Institute’s division of cancer control and population sciences, said the findings from the latest study must be borne out in further research. But in the meantime, making dietary changes that reduce glycemic load is a reasonable recommendation for colon cancer patients, he said, since it can only be helpful, not harmful.
“The results of the current study need to be confirmed, but the current indications are that diet is important to colon cancer survival,” Dr. Mahabir said.
For ancient Maya, Climate change may have been blessing and curse
The ancient Maya civilization may have risen — and then fallen — in response to climate change, scientists report after creating precise climate records going back 2,000 years.
The researchers, whose findingsappear in the current issue of the journal Science, reconstructed rainfall patterns using cross-sections of stalagmites from a cave near the ancient city of Uxbenka, in what is now southern Belize. First, they dated the samples with the technique called uranium-thorium dating; then, to generate a climate record, they measured oxygen isotopes, which are sensitive to rainfall.
- The early classic Maya period — about A.D. 450 to 660 — “was remarkably wet,” said an author of the study, Douglas Kennett, a geo-archaeologist at Penn State. “There was a proliferation of population, an increase in agriculture and a rise in divine kings that became prominent leaders.”
- But then things dried up. The researchers compared the climate record with an existing “war index” — a log of hostile events based on how often certain keywords occurred in Maya inscriptions on stone monuments — and found a strong correlation between drought and warfare between cities.
- “About A.D. 660, you get indications of some social stress that goes up in tandem with this drying period,” Dr. Kennett said.
- Maya cities were linked, but each operated with its own autonomous political structure. When resources were strained, the groups may have turned against one another.
- Over several hundred years, “the social fabric was eventually destabilized,” Dr. Kennett said. Most Maya cities collapsed between A.D. 800 and 900.
How to devise passwords that drive hackers away
Not long after I began writing about cyber security, I became a paranoid caricature of my former self. It’s hard to maintain peace of mind when hackers remind me every day, all day, just how easy it is to steal my personal data.
Within weeks, I set up unique, complex passwords for every Web site, enabled two-step authentication for my e-mail accounts, and even covered up my computer’s Web camera with a piece of masking tape — a precaution that invited ridicule from friends and co-workers who suggested it was time to get my head checked.
But recent episodes offered vindication. I removed the webcam tape — after a friend convinced me that it was a little much — only to see its light turn green a few days later, suggesting someone was in my computer and watching. More recently, I received a text message from Google with the two-step verification code for my Gmail account. That’s the string of numbers Google sends after you correctly enter the password to your Gmail account, and it serves as a second password. (Do sign up for it.) The only problem was that I was not trying to get into my Gmail account. I was nowhere near a computer. Apparently, somebody else was.
It is absurdly easy to get hacked. All it takes is clicking on one malicious link or attachment. Companies’ computer systems are attacked every day by hackers looking for passwords to sell on auctionlike black market sites where a single password can fetch $20. Hackers regularly exploit tools like John the Ripper, a free password-cracking program that use lists of commonly used passwords from breached sites and can test millions of passwords per second.
Chances are, most people will get hacked at some point in their lifetime. The best they can do is delay the inevitable by avoiding suspicious links, even from friends, and manage their passwords. Unfortunately, good password hygiene is like flossing — you know it’s important, but it takes effort. How do you possibly come up with different, hard-to-crack passwords for every single news, social network, e-commerce, banking, corporate and e-mail account and still remember them all?
To answer that question, I called two of the most (justifiably) paranoid people I know, Jeremiah Grossman and Paul Kocher, to find out how they keep their information safe. Mr. Grossman was the first hacker to demonstrate how easily somebody can break into a computer’s webcam and microphone through a Web browser. He is now chief technology officer at WhiteHat Security, an Internet and network security firm, where he is frequently targeted by cybercriminals. Mr. Kocher, a well-known cryptographer, gained notice for clever hacks on security systems. He now runs Cryptography Research, a security firm that specializes in keeping systems hacker-resistant. Here were their tips:
FORGET THE DICTIONARY If your password can be found in a dictionary, you might as well not have one. “The worst passwords are dictionary words or a small number of insertions or changes to words that are in the dictionary,” said Mr. Kocher. Hackers will often test passwords from a dictionary or aggregated from breaches. If your password is not in that set, hackers will typically move on.
NEVER USE THE SAME PASSWORD TWICE People tend to use the same password across multiple sites, a fact hackers regularly exploit. While cracking into someone’s professional profile on LinkedIn might not have dire consequences, hackers will use that password to crack into, say, someone’s e-mail, bank, or brokerage account where more valuable financial and personal data is stored.
COME UP WITH A PASSPHRASE The longer your password, the longer it will take to crack. A password should ideally be 14 characters or more in length if you want to make it uncrackable by an attacker in less than 24 hours. Because longer passwords tend to be harder to remember, consider a passphrase, such as a favorite movie quote, song lyric, or poem, and string together only the first one or two letters of each word in the sentence.
OR JUST JAM ON YOUR KEYBOARD For sensitive accounts, Mr. Grossman says that instead of a passphrase, he will randomly jam on his keyboard, intermittently hitting the Shift and Alt keys, and copy the result into a text file which he stores on an encrypted, password-protected USB drive. “That way, if someone puts a gun to my head and demands to know my password, I can honestly say I don’t know it.”
STORE YOUR PASSWORDS SECURELY Do not store your passwords in your in-box or on your desktop. If malware infects your computer, you’re toast. Mr. Grossman stores his password file on an encrypted USB drive for which he has a long, complex password that he has memorized. He copies and pastes those passwords into accounts so that, in the event an attacker installs keystroke logging software on his computer, they cannot record the keystrokes to his password. Mr. Kocher takes a more old-fashioned approach: He keeps password hints, not the actual passwords, on a scrap of paper in his wallet. “I try to keep my most sensitive information off the Internet completely,” Mr. Kocher said.
A PASSWORD MANAGER? MAYBE Password-protection software lets you store all your usernames and passwords in one place. Some programs will even create strong passwords for you and automatically log you in to sites as long as you provide one master password. LastPass, SplashData and AgileBits offer password management software for Windows, Macs and mobile devices. But consider yourself warned: Mr. Kocher said he did not use the software because even with encryption, it still lived on the computer itself. “If someone steals my computer, I’ve lost my passwords.” Mr. Grossman said he did not trust the software because he didn’t write it. Indeed, at a security conference in Amsterdam earlier this year, hackers demonstrated how easily the cryptography used by many popular mobile password managers could be cracked.
IGNORE SECURITY QUESTIONS There is a limited set of answers to questions like “What is your favorite color?” and most answers to questions like “What middle school did you attend?” can be found on the Internet. Hackers use that information to reset your password and take control of your account. Earlier this year, a hacker claimed he was able to crack into Mitt Romney’s Hotmail and Dropbox accounts using the name of his favorite pet. A better approach would be to enter a password hint that has nothing to do with the question itself. For example, if the security question asks for the name of the hospital in which you were born, your answer might be: “Your favorite song lyric.”
USE DIFFERENT BROWSERS Mr. Grossman makes a point of using different Web browsers for different activities. “Pick one browser for ‘promiscuous’ browsing: online forums, news sites, blogs — anything you don’t consider important,” he said. “When you’re online banking or checking e-mail, fire up a secondary Web browser, then shut it down.” That way, if your browser catches an infection when you accidentally stumble on an X-rated site, your bank account is not necessarily compromised. As for which browser to use for which activities, a study last year by Accuvant Labs of Web browsers — including Mozilla Firefox, Google Chrome and Microsoft Internet Explorer — found that Chrome was the least susceptible to attacks.
SHARE CAUTIOUSLY “You are your e-mail address and your password,” Mr. Kocher emphasized. Whenever possible, he will not register for online accounts using his real e-mail address. Instead he will use “throwaway” e-mail addresses, like those offered by10minutemail.com. Users register and confirm an online account, which self-destructs 10 minutes later. Mr. Grossman said he often warned people to treat anything they typed or shared online as public record.
“At some point, you will get hacked — it’s only a matter of time,” warned Mr. Grossman. “If that’s unacceptable to you, don’t put it online.”
The Science and Art of Listening
If your home is like mine, you hear the humming sound of a printer, the low throbbing of traffic from the nearby highway and the clatter of plastic followed by the muffled impact of paws landing on linoleum — meaning that the cat has once again tried to open the catnip container atop the fridge and succeeded only in knocking it to the kitchen floor.
The slight trick in the question is that, by asking you what you were hearing, I prompted your brain to take control of the sensory experience — and made you listen rather than just hear. That, in effect, is what happens when an event jumps out of the background enough to be perceived consciously rather than just being part of your auditory surroundings. The difference between the sense of hearing and the skill of listening is attention.
Hearing is a vastly underrated sense. We tend to think of the world as a place that we see, interacting with things and people based on how they look. Studies have shown that conscious thought takes place at about the same rate as visual recognition, requiring a significant fraction of a second per event. But hearing is a quantitatively faster sense. While it might take you a full second to notice something out of the corner of your eye, turn your head toward it, recognize it and respond to it, the same reaction to a new or sudden sound happens at least 10 times as fast.
This is because hearing has evolved as our alarm system — it operates out of line of sight and works even while you are asleep. And because there is no place in the universe that is totally silent, your auditory system has evolved a complex and automatic “volume control,” fine-tuned by development and experience, to keep most sounds off your cognitive radar unless they might be of use as a signal that something dangerous or wonderful is somewhere within the kilometer or so that your ears can detect.
This is where attention kicks in.
Attention is not some monolithic brain process. There are different types of attention, and they use different parts of the brain. The sudden loud noise that makes you jump activates the simplest type: the startle. A chain of five neurons from your ears to your spine takes that noise and converts it into a defensive response in a mere tenth of a second — elevating your heart rate, hunching your shoulders and making you cast around to see if whatever you heard is going to pounce and eat you. This simplest form of attention requires almost no brains at all and has been observed in every studied vertebrate.
More complex attention kicks in when you hear your name called from across a room or hear an unexpected birdcall from inside a subway station. This stimulus-directed attention is controlled by pathways through the temporoparietal and inferior frontal cortex regions, mostly in the right hemisphere — areas that process the raw, sensory input, but don’t concern themselves with what you should make of that sound. (Neuroscientists call this a “bottom-up” response.)
But when you actually pay attention to something you’re listening to, whether it is your favorite song or the cat meowing at dinnertime, a separate “top-down” pathway comes into play. Here, the signals are conveyed through a dorsal pathway in your cortex, part of the brain that does more computation, which lets you actively focus on what you’re hearing and tune out sights and sounds that aren’t as immediately important.
In this case, your brain works like a set of noise-suppressing headphones, with the bottom-up pathways acting as a switch to interrupt if something more urgent — say, an airplane engine dropping through your bathroom ceiling — grabs your attention.
Hearing, in short, is easy. You and every other vertebrate that hasn’t suffered some genetic, developmental or environmental accident have been doing it for hundreds of millions of years. It’s your life line, your alarm system, your way to escape danger and pass on your genes. But listening, really listening, is hard when potential distractions are leaping into your ears every fifty-thousandth of a second — and pathways in your brain are just waiting to interrupt your focus to warn you of any potential dangers.
Listening is a skill that we’re in danger of losing in a world of digital distraction and information overload.
And yet we dare not lose it. Because listening tunes our brain to the patterns of our environment faster than any other sense, and paying attention to the nonvisual parts of our world feeds into everything from our intellectual sharpness to our dance skills.
Luckily, we can train our listening just as with any other skill. Listen to new music when jogging rather than familiar tunes. Listen to your dog’s whines and barks: he is trying to tell you something isn’t right. Listen to your significant other’s voice — not only to the words, which after a few years may repeat, but to the sounds under them, the emotions carried in the harmonics. You may save yourself a couple of fights.
“You never listen” is not just the complaint of a problematic relationship, it has also become an epidemic in a world that is exchanging convenience for content, speed for meaning. The richness of life doesn’t lie in the loudness and the beat, but in the timbres and the variations that you can discern if you simply pay attention.
